Lost Passport

If you happen to misplace your passport or if it was stolen, then it is important to report it to the U.S. Department of State in order to protect your identity. Bounce Visa Services will ease your mind by helping you protect your identity and obtain your lost passport replacement.

To replace a lost or stolen passport, you will need to apply for a new passport and include a special form stating what happened to your original passport. When applying for a new passport, you are required to go to a local passport acceptance agent to prove your identity. Then Bounce Passport & Visa can take it from there and get your lost passport replacement quickly.
Report Lost Passport
Currently, there are two ways to report a lost passport to the U.S. Department of State. You can either complete the Statement Regarding Stolen or Lost Passport with the Form DS-64. Or, call the National Passport Information Center.
FORM DS-64
You should fill out the DS-64 form only if your lost passport is still valid. Answer as many questions that you can with as much detail as you can. Once you have completed the form you should mail it to the address below.

National Passport Information Center
The NPIC is a call center that answers questions and provides information for American citizens who need to apply or have applied for passport services. The toll-free number is 1-877-487-2778. While calling may seem to be the more expedient of the two methods, it is often very difficult to get through.
Complete Application Form
One of the criteria that you must meet in order to apply for a passport renewal by mail is that you have to be able to submit your previously-issued, undamaged passport. Applicants who have lost their travel document are unable to submit them and, therefore, must apply for a new passport using Form DS-11.
FORM DS-11
Just like reporting the lost passport, you have two options for completing the application form. One, you can complete Form DS-11 online or you can complete the application for a new U.S. passport offline and then print it.
Gather Supporting Documents
All documents required for a first-time passport will have to be submitted together with the completed application in order to replace your passport. Following is a list of the documents you will need.
Evidence of U.S. citizenship
Proof of identity
Photocopy of the identification document
One recent passport photo
Visit Application Acceptance Facility
The completed application form and supporting documents must be submitted to an authorized acceptance agent at an application acceptance facility.
Applications that are submitted at a passport office take up to six weeks to process unless you request expedited service in which case it could arrive in 2 or 3 weeks. Bounce Passport and Visa will get your child's passport as quickly as you need it, according to the service you pick.
