Name Change

If your name has been legally changed since your travel document was issued, you should apply to change it. The application and supporting documents that you will submit depend on a couple of factors so read carefully to make sure you are submitting the correct ones.

Complete Name Change Application Form
There are three possible applications forms that can be used to change the name in a passport. Choose which form you need to complete. You can download it and obtain instructions and tips for completing the application.
Form DS-11 No document to prove legal name change
Form DS-82 Passport issued more than one year ago
Form DS-5504 Passport issued less than one year ago
Gather Supporting Documents
The document submitted as evidence of the legal name change must be an original or a certified copy. Acceptable documents include a marriage certificate, divorce decree, adoption certificate or a court order.
Send Name Change Dossier
Both Forms DS-5504 and DS-82 can be mailed to the address on the applications. Place the completed applications, supporting documents and payment for fees in a protective packaging. It is also recommended that you use a traceable delivery method. If you need to use Form DS-11, you are required to appear in person at an application acceptance facility.

Bounce Passport and Visa is a Passport Courier Service. We charge an additional fee to hand-carry passport applications to the Passport Agency for people who value their time and do not want to do it themselves. You can drop-off and pick-up your new passport at our Houston Midtown Office and never stand in line.

If you do not need our service, you can apply directly with US Department of State.
